MO512THE RENAL CONSEQUENCES OF LUNG TRANSPLANTATION IN ADULT CYSTIC FIBROSIS PATIENTS

Abstract Background and Aims Cystic fibrosis patients are at high risk for chronic kidney disease (CKD), because of aminoglycoside exposure, protein dietary salt intake, Fibrosis Related Diabetes. Lung transplantation also entails a renal burden in the long run calcineurin inhibitors exposure. We aimed studying function by gold standard glomerular filtration rate (mGFR) before 1 year after lung transplantation, as well identify factors impairment. Method Patients from reference center Lyon, France were systematically referred to nephrology department transplantation. mGFR (iohexol or inulin clearance) was performed 12-18 months Risk worse nephrological outcome (i.e. post < 60mL/min/1.73m²) searched for. Results 28 (10 males), mean age 32 yr, underwent pre mGFR. Three died following two progressed end stage rapidly thereafter. 23 benefited both Mean 105±21mL/min/1.73m² 67±19 mL/min/1.73m² transplant respectively. Factors associated with univariate multivariate analysis (p=0.03) occurrence acute injury post-operative period (p=0.005). Cumulated exposure time spent under cardiopulmonary bypass not outcome. Conclusion is adult patients. advise clinical referral nephrologist part systematic evaluation candidates even if GFR normal range.
